Today is my S&W M&P 340 with Hornady .38 SPCL +P 110 gr Critical Defense inside a cheap Galco IWB carried cross-draw and my NAA .22 mini loaded with SSS 60 gr .22LR in my right front pocket.

Some days I carry just my NAA, some days my M&P inside a homemade pocket holster in my left front pocket and some days my PPK/s either in a Bucheimer OWB or in a Gunskin IWB.

With the cooler temps I am more able to wear either OWB or IWB and not as much pocket carry as during the hotter months.

  Usually the nickle S&W model 38 carried in the old Courtland holster on a regular dress belt worn crossdraw, under a t-shirt, polo shirt or sweatshirt.
  If, depending upon my destination, time of day, manner of dress, and several other criteria it may be the Glock 27 in that El Paso Saddlery 'High Slide'. Both are equipped with alternate methods of carry, the Smith wears some Barami grips and the Glock a Clipdraw.

(have not found a mag carrier that I like).

+1.  A good mag carrier has me stumped, too.

I have three holsters for my Glock 23:  A safepacker, a Blackhawk SERPA paddle, and a Crossbreed supertuck. 
Carry ammo is TAP or the Fiochi copy of TAP's.

Cover garments:
Duluth Trading sportcoat (it is a perfect mix of a sportcoat and an old Carhartt barncoat.)
Patagonia micropuff vest (the granola logo is great diversion).
Aloha shirt.

My EDC is an M&P 9mm full-sized in a TT Gunleather IWB. Soon it'll be a .45 M&P full-sized as the weather is getting cooler and the potential assailants cover garments are getting heavier.

Two spare mags in my left pocket (have not found a mag carrier that I like).
Here's a couple links for mag holders I really like the desantis it keeps my spare full-size mag properly orientated and dust bunny free in my pocket.

For me these days I usually pack a M&P9c in a Crossbreed Supertuck, or a Colt Mustang Pocket-lite .380 in a Kramer pocket Holster.

Normal,  J frame 9mm strong pocket galco leather pocket holster, 2 moon clips left pants pocket. 2 flashlights, 1 emt knife, one folding pigsticker.
