Why?

From the FTFs and the constant use witht he second mag of the charging handle and forward assist - especially since he felt he needed to push the charging handle forward (? ? ?), something was clearly wrong.

I don't think it had anything to do with the forward assist.  I think he was just hitting it because he has been told that is the way to us an AR.

He had a miss fire with the first mag, and then the series with the second mag.  I think he had a bad reloading experience.

Did you also notice that Einstein determined that these were all bad primers simply by racking the action?

I had a similar thing happen at a match.  For me it was over pressure loads.  Cost me $500 to rebuild the upper.

I think he had a broken part in the bolt which prevented the bolt locking up, which caused the failure to fire.  He forced the carrier in position but it did not lock up.

When he fired the cartriage was not completely in place and ruptured possibly breaking the bolt and carrier.

The blast focused down through the magazine as it should.  He is a very luck knuckle head.

hit the forward assist, pulled the charging handle, then tapped the bottom of the mag, WTF ?
right actions, wrong order, the forward assist had nothing to do with his Kaboom as the round he "assisted" was then ejected.
With his crappy IAD technique  it is possible that the the bolt wasn't fully seated.

I don't know shit about AR's, BUT, I can recognize an IDIOT from way back here!  

This guy should never be given a firearm without proper and extensive supervision and training.

Maybe I'm wrong, but I thought an AR firing pin COULDNT reach the primer until the locking lugs were in place...  :-\

The bolt kinda "cams" around and moves back into the bolt carrier so the firing pin reaches right?

Soooooo, the explosion had to happen "in battery".  Sooooo, even with a WAYYYY overpressured round you couldnt blow the gun up like this without a clogged barrel?  JUST GUESSING!  :-\
