S&W Bodyguard Pistol - Barrel & Slide Ejection Feature

I haven't had much time to be on the site so I'm not sure if this subject has been discussed. I had a customer who purchased a S&W Bodyguard pistol from the shop a week ago. He came in the shop and I asked him how he likes it so far. He said he was shooting it last weekend at an outdoor range and he was into his second box of ammo when he fired it and the slide and barrel flew forward off the gun.
The take down pin works itself loose while shooting and it disengaged (fell off) resulting in the the slide and barrel to eject forward on a shot. We sent his back to S&W.

I was talking to a shop owner a couple of weeks ago about one and he said that out of the 5 he had sold, 3 had to be sent back for broken triggers. This was all around 50 - 60 rounds going through the guns. I decided to pass and picked up a Bond Arms Snake Slayer instead.

UPDATE:
Smith & Wesson is sending my customer a new Bodyguard 380 to replace the one with the barrel and slide ejection feature.
We got a new batch of them in the shop and one noticeable thing Smith did was change the frame mold to have a raised "O" ring around the laser activation button. The first models did not have this and the lasers could turn on while in a pocket or holster draining the battery.

My customer gave me an update on the replacement Bodyguard he got from Smith. The replacement has light primer strikes. He tried three different ammo manufacturers and the problem happened with all three. It goes bang only about 50% of the time. He is not happy with Smith right now.
